The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The focus of IUIH Aged Care team is to work with the local Indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services to eligible aged care clients.
About the Role?
This position plays a key role in an innovative pilot program delivering culturally safe, person-centred aged care assessment services to older Aboriginal and Torres Strait Islander people operating in one or both Caboolture and Sunshine Coast Aged Care Planning Regions (ACPRs). As an Elder Care and Assessment Connector, your primary responsibilities are to connect clients and their representatives/families to raise awareness of their aged care entitlements, to assist their entry into the aged care system and to facilitate their access to culturally safe needs assessments for aged care services. The overarching function of this position is to ensure that support for Elders is organised, sequenced and supported to enable effective access to and assessment for appropriate aged care services.
This role is a full-time temporary role available.. Within this role, you will be required to:
- Assist Aboriginal and Torres Strait islander older persons with advocacy and support throughout their My Aged Care journey
- Support clients to identify goals and needs to ensure they get the appropriate level of culturally safe care and services, which can include referrals to external services and support.
- Deliver active community outreach to promote aged care services, and identify and engage with potential clients.
- Support clients and their family to identify and select appropriate aged care service providers, understand contractual arrangements and costs of care, and ensure the client's care and cultural safety needs are met.
About You?
- Current C Class Drivers Licence (QLD)
- Current Criminal History Check - required prior to your commencement
- Current Disability Workers Screening Card (QLD Yellow Card) - required prior to your commencement
Desirable, but not mandatory:
- Knowledge of Aboriginal and Torres Strait Island culture through either identifying as Aboriginal or Torres Strait Island person and/or demonstrated experience of working successfully within Aboriginal and Torres Strait Island communities.
- Knowledge of current Australian Aged Care System reforms and access, assessment, service delivery arrangements.
