About Us?
The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The focus of IUIH Aged Care team is to work with the local Indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services to eligible aged care clients.

About the Role?
This position plays a key role within an innovative pilot program delivering culturally safe, person-centred aged care Assessment services Assessment Services to older Aboriginal and Torres Strait Islander people, operating in one or both of the Caboolture and Sunshine Coast Aged Care Planning Regions (ACPRs). The role is responsible for conducting culturally appropriate Home Support Assessments for eligible older Aboriginal and Torres Strait Islander individuals, in accordance with the policies and procedures of the Aged Care Single Assessment System.

We have a full-time temporary role available. Within this role, you will be required to:

• Acquire and maintain a thorough understanding of the Assessment policy and procedure documents which govern the Aged Care Single Assessment System
• Comply with IUIH’s Standard Operating Procedures for the Pilot.
• Undertake assessments as allocated by the Team Leader, using the Integrated Assessment Tool (IAT), of older Aboriginal and Torres Strait Islander people which do not require clinical judgement, in line with My Aged Care and IUIH procedures
• Complete Support Plan Reviews referred from the Team Leader.
• Update client information in My Aged Care to keep records accurate and up to date.
  
  

About You?

• Preferably, one year demonstrated RAS aged care assessment experience in Australia.
• Current C Class Drivers Licence (QLD)
• Current Criminal History Check - required prior to your commencement
• Current Disability Workers Screening Card (QLD Yellow Card) – required prior to your commencement
  
  

Desirable, but not mandatory:

• Knowledge of Aboriginal and Torres Strait Island culture through either identifying as Aboriginal or Torres Strait Island person and/or demonstrated experience of working successfully within Aboriginal and Torres Strait Island communities.
• Knowledge of current Australian Aged Care System reforms and access, assessment, service delivery arrangements.
