The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The focus of IUIH Aged Care team is to work with the local Indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services to eligible aged care clients.
About the Role?
This position plays a key role within an innovative pilot program delivering culturally safe, person-centred aged care Assessment services to older Aboriginal and Torres Strait Islander people, operating in one or both Caboolture and Sunshine Coast Aged Care Planning Regions (ACPRs). The position is to provide leadership to a multi-disciplinary team delivering the innovative Assessment Services to older Aboriginal and Torres Strait Islander people (Pilot). You will be responsible for overseeing home support and Comprehensive Aged Care Assessments of Aboriginal and Torres Strait Islander older persons while complying with policies and procedures of the Aged Care Single Assessment System.
We have a full-time temporary role available. Within this role, you will be required to:
- Acquire and maintain a thorough understanding of the Assessment policy and procedure documents which govern the Aged Care Single Assessment System
- Manage referrals following the Best Practice Steps outlined in the My Aged Care Assessment Manual.
- Assess referrals and determine the appropriate outcome
- Manage and oversee the activities of aged care assessors and ensure that all mandatory training required for the Aboriginal and Torres Strait Islander Aged Care Assessment Pilot is completed by assessors within required timeframes.
- Liaise with the Administration Officer, ensure rostering and scheduling of assessment services is logistically sound, efficient and effective use of staffing resources.
About You?
- Current C Class Drivers Licence (Qld)
- Current Criminal History Check – required prior to commencement
- Current Disability Worker Screening Check (QLD Yellow Card) – required prior to commencement
- Tertiary qualification in a health-related discipline directly related to health, aged care or related specialist area.
- Current unrestricted APHRA Registration
- At least one year's Aged Care Needs Assessment experience is preferred.
- Demonstrated experience in the coordination of Aged Care service delivery and/or Aged Care Assessment Teams
Desirable, but not mandatory:
- Knowledge of Aboriginal and Torres Strait Island culture through either identifying as Aboriginal or Torres Strait Island person and/or demonstrated experience of working successfully within Aboriginal and Torres Strait Island communities.
- Knowledge of current Australian Aged Care System reforms and access, assessment, service delivery arrangements.
