About the Role
As a Care Manager Community Nurse, you will take a leading role in coordinating and delivering comprehensive, evidence-based nursing care to clients in their homes. You will build meaningful relationships with clients and families, develop care plans that reflect their values and preferences, and ensure continuity of care through skilled case management. This role is ideal for a nurse who is:
Ready to step into a more autonomous, community-based role
Interested in building long-term relationships with clients
Passionate about aged care, chronic disease management, and discharge support
Key Responsibilities:
Provide high-quality clinical care including wound care, continence management, medication administration, falls prevention, and health assessments
Develop, implement, and review individualized care plans that support clinical goals and personal aspirations
Coordinate care transitions (e.g., hospital discharges), ensuring clients receive timely and effective support at home
Collaborate with GPs, hospitals, allied health professionals, Nurse Navigators, and internal QAADS team members to provide integrated care
Complete documentation and reporting in accordance with quality and compliance standards
Mentor and support care workers to ensure service delivery aligns with clinical best practices
Contribute to the development and improvement of policies and procedures across aged care services
Essential:
Current AHPRA registration as a Registered Nurse
Experience working in multidisciplinary healthcare teams
Proven expertise in hospital discharge planning, case management, and client risk assessments
Strong clinical reasoning, problem-solving, and communication skills
Ability to manage competing priorities in a fast-paced, client-centered environment
Hold a current QLD Driver License and willing to obtain police checks as required.
Desirable:
Minimum of 2 years’ experience in aged care or home care services
Familiarity with aged care regulations and quality standards
