Clinical Nurse Coordinator | Clinical Nurse Manager - Residential Aged Care | Perth Metro

We are currently working with a highly regarded not-for-profit aged care provider seeking a dedicated Clinical Coordinator to join a brand new, modern, purpose-built residential aged care facility located in the East Perth metropolitan area.

This is a fantastic opportunity for a highly experienced Registered Nurse or CCC/ CNM to step into a key leadership role within a values-driven organisation committed to delivering high-quality, person-centred care.

About the Role:

Reporting to the Facility Manager and working alongside another Clinical Nurse Coordinator, you will play a vital role in managing daily clinical operations, mentoring staff, and ensuring compliance with all regulatory and accreditation standards.

Key Responsibilities:

Oversee the coordination of daily clinical care and care planning

Lead family conferences and maintain positive resident and family relationships

Support AN-ACC assessments and ensure accurate clinical documentation

Contribute to ongoing quality improvement and participate in internal audits

Identify staff training needs and support professional development

Manage clinical stock levels and equipment to support effective care delivery

Respond to clinical incidents and feedback to ensure continuous improvement

What We're Looking For:

Current AHPRA registration as a Registered Nurse

Demonstrated clinical leadership experience, ideally in aged care

Strong understanding of Aged Care Quality Standards and AN-ACC requirements

Ability to lead, mentor and support care teams

Excellent communication and interpersonal skills

Commitment to person-centred care and resident wellbeing
