Moorditj Koort Aboriginal Corporation (MKAC) is a community-controlled, not-for-profit organisation dedicated to supporting the health and wellbeing of Aboriginal and Torres Strait Islander peoples across Western Australia. Our vision is to see Aboriginal communities flourish through access to holistic, culturally safe health and wellbeing services.

We are seeking an experienced Clinical Registered Nurse (Clinic & TCP Aged Care) who will provide high-quality, person-centred, and culturally safe nursing care across both our primary health clinic and Transition Care Program (TCP). This dual role supports Elders and community members through acute care, chronic disease management, and aged care services.

About the Role

The Clinical Registered Nurse will work in MKAC's clinic while also supporting aged care clients through the Transition Care Program. This role involves direct clinical care, health promotion, and chronic disease management, while also providing leadership and care coordination to ensure Elders and patients receive holistic, culturally appropriate services.

Key Responsibilities
Clinical Care – Clinic & TCP

Conduct comprehensive health assessments, including physical examinations and care planning.

Manage acute and chronic health conditions across both clinic and aged care settings.

Administer medications, vaccinations, and treatments in accordance with clinical best practice.

Provide wound care, palliative care, and end-of-life support with cultural sensitivity.

Support Elders transitioning from hospital to home (TCP), ensuring continuity of care.

Chronic Disease & Aged Care Management

Develop and oversee care plans for patients with chronic diseases and aged care needs.

Monitor and evaluate care plans, adjusting in collaboration with multidisciplinary teams.

Deliver patient and family education on health maintenance, healthy ageing, and disease prevention.

Advocate for Elders' health and wellbeing across community, hospital, and aged care systems.

Collaboration & Care Coordination

Work closely with GPs, Aboriginal Health Workers, allied health, and aged care providers.

Coordinate referrals and services to ensure seamless care pathways.

Maintain accurate clinical documentation using MMEx and organisational systems.

Contribute to quality improvement initiatives and clinical governance processes.

Essential Requirements

Current AHPRA Registration as a Division 1 Registered Nurse.

Bachelor of Nursing (or equivalent).

Demonstrated clinical experience in aged care, gerontology, or TCP programs.

Minimum of 2 years' clinical experience in aged care, gerontology, or Transition Care Program.

Aged care experience is mandatory for this role.

Knowledge of Aged Care Quality Standards and person-centred care principles.

Valid WA Driver's Licence.

National Police Clearance (within 3 months).

Current Working with Children Check.

First Aid Certificate (or willingness to obtain).

COVID-19 Vaccination Certificate.

Aboriginal or Torres Strait Islander heritage is preferred for this position.

Skills & Attributes

Strong clinical knowledge and ability to work confidently across both primary health and aged care.

Experience supporting Elders through hospital discharge, rehabilitation, and transition care.

Ability to mentor and work collaboratively with Aboriginal Health Workers and care teams.

Excellent interpersonal skills, with a compassionate and culturally safe approach.

Strong organisational and problem-solving skills, with the ability to manage competing priorities.

Proficiency in Microsoft Office and electronic health systems (MMEx preferred).
