We’re on the lookout for a passionate NDIS Support Coordinator to help participants navigate their plans, connect with providers, and achieve their personal goals. You will also be managing Day Programs and school holidays activities for our participants.

What You’ll Do:

Working collaboratively with participants, their families and support networks.

Assisting Participants to gather formal documentation to gain access to the NDIS (National Disability Insurance Scheme).

Supporting participants to achieve their goals through strength-based and person-centred planning.

Ensuring the delivery of services in accordance with relevant Standards, Legislation and other regulatory or contractual requirements associated with program funding and contribute to reporting as required.

Manage a diverse caseload with flexibility and purpose

Day Program/Group Home activities/ School Holidays

Home visit to participants

Rostering, leading and inspiring a team of local Support Workers to deliver exceptional services.

Ensure service operates within allocated budget.

What You’ll Bring:

Bachelor of relevant field

NDIS Worker Check and WWCC (Mandatory)

Driver’s licence and own a car

Has had experience in a NDIS Support Coordination role,

Experience in running Day Program/Group Home activities/ School Holidays

Knowledge of the NDIS Compliance

Strong customer service skills with demonstrated ability to develop and maintain relationships with people with disabilities and/ or their families and support network and service providers.

Proven experience in developing productive networks and working relationships.

Sound knowledge and experience with the National Disability Insurance Scheme.

Ability to prioritise work, manage time and meet deadlines
