The Opportunity
- Role: NDIS Support Coordinator
- Full Time: 38 hours a week
- Days: Monday to Friday
- Location: Shoalhaven
- Hourly Pay Rate: $44.58
We are seeking an enthusiastic NDIS Support Coordinator for a full-time role to ensure the planning and delivery of flexible, integrated, and timely services for NDIS participants. You will leverage your knowledge of both funded and unfunded supports to oversee coordination in alignment with participants’ NDIS plans. Our collaborative team upholds Flagstaff’s core values and is dedicated to providing a person-centered approach.

Key Responsibilities & Requirements:
- Provide support in line with National Quality & Safeguards standards for people with disability
- Demonstrate excellent knowledge of local disability services (funded and non-funded)
- Oversee assessments, reports, and service proposals aligned with individual participant goals
- Apply Person-Centered Approaches to empower participant choice and community involvement
- Interpret NDIS plans and establish SMART goals within Support Coordination
- Deliver timely, sound advice to internal and external stakeholders
- Build strong relationships with participants, families, carers, and service providers
- Respond calmly and professionally to crises, using strong problem-solving skills
- Maintain accurate, timely records, reports, and database entries
- Facilitate participant transport when required
- Provide quality advice on NDIS processes, evidence recording, and internal procedures
The Ideal Candidate
- Minimum 12 months experience in a Support Coordination role, LAC, NDIS planner, or similar.
- Minimum Cert IV in Disability or other relevant qualifications.
- Experience in interpreting and implementing individual NDIS plans.
- Familiarity with working alongside individuals living with a disability, addressing their communication and cultural needs.
- Strong verbal & written communication, interpersonal, and negotiation skills.
- Proven experience in record keeping, with a focus on confidentiality and security of information.
- Current driver’s license and access to a fully insured vehicle.
- Ability to manage time effectively, working both autonomously and as part of a team.

- Hourly rate based on SCHADS Award - Level 4, Pay point 1 - at $44.58 plus 12% superannuation
