Our team is passionate about empowering individuals with disabilities to live fulfilling lives. We are seeking a dedicated and compassionate NDIS Support Coordinator to join our growing team.
Position Overview:
As an NDIS Support Coordinator, you will assist participants in understanding and implementing their NDIS plans, connecting with service providers, and building capacity to manage their supports independently. You will play a key role in ensuring participants receive the right support at the right time, while maintaining compliance with NDIS guidelines.
Key Responsibilities:
Support Coordination: Help participants understand their NDIS plans and connect with appropriate services and supports.
Capacity Building: Empower participants to manage their own supports and make informed decisions.
Service Engagement: Liaise with service providers, community organisations, and government agencies to coordinate supports.
Plan Implementation: Monitor progress and ensure services are delivered in line with participant goals.
Documentation & Reporting: Maintain accurate records and prepare reports in accordance with NDIS requirements.
Crisis Support: Assist participants in navigating complex or crisis situations and develop risk management strategies.
Compliance & Quality Assurance: Ensure all coordination activities meet NDIS standards and internal policies.
What We’re Looking For:
Education: Relevant qualification in disability, social work, psychology, human services, or related field.
Experience: Minimum of 2 years in support coordination or case management within the NDIS or community services sector.
Skills: Excellent communication, problem-solving, and organisational skills. Ability to work independently and collaboratively. Familiarity with NDIS systems and portals.
Attributes: Empathetic, proactive, and committed to person-centred practice. Strong attention to detail and ability to manage multiple priorities.
Preferred Qualifications:
Experience working with participants with complex needs.
Knowledge of NDIS legislation and operational guidelines.
Experience using client management systems (e.g., VisualCare, Shiftcare).
Strong stakeholder engagement and advocacy skills.
