About the Role

Journey Connect is seeking a dedicated and dynamic Service Delivery Specialist to join our team. This role is pivotal in ensuring exceptional service delivery to our participants under the National Disability Insurance Scheme (NDIS).

As a Service Delivery Specialist, you will be responsible for:

Client Onboarding: Facilitating seamless onboarding processes, including writing comprehensive care plans and risk plans tailored to individual participant needs.

Rostering: Matching suitably qualified and experienced staff with participants to ensure optimal support delivery.

Database Management: Maintaining accurate and up-to-date client and staff records in our database systems.

Quality Management: Ensuring supports are delivered with a focus on improving participants' lives, maintaining high standards of care and service quality.

Compliance: Ensuring staff adherence to NDIS frameworks and Journey Connect's policies and procedures.

Collaboration: Building and maintaining quality relationships with stakeholders, including families, caregivers, coordinators, medical professionals, and others connected to participants.

On-Call Duties: Being available for roster or shift changes to ensure continuity of service.

Training: Conducting staff inductions and ongoing training to support quality service delivery.

Effectively, you will know both our clients and our staff and be the central hub to ensure that quality services are delivered to our participants.

About You

We are looking for a candidate with the following skills and attributes:

Organisational Skills: Exceptional ability to manage multiple tasks, including client onboarding, rostering, and database management.

Communication Skills: Strong verbal and written communication skills to build relationships with stakeholders and create clear, detailed care and risk plans.

Attention to Detail: A meticulous approach to ensure compliance with NDIS frameworks and Journey Connect policies.

Problem-Solving: Ability to address roster changes and participant needs efficiently, with a proactive and solution-focused mindset.

Training and Leadership: Experience in training and mentoring staff to maintain high standards of service delivery.

Empathy and Interpersonal Skills: A compassionate approach to working with participants, families, and stakeholders to foster trust and collaboration.

Technical Proficiency: Familiarity with database systems and tools for managing client and staff information. You will be trained in our specific tools but an understanding of the Microsoft environment is essential.

Proficiency in Microsoft Office Suite (Word, Excel, Outlook) is essential. Experience with CRM systems, case management software, or similar database platforms preferred.

Flexibility: Willingness to be on-call and adapt to changing schedules or participant requirements. This is an additional allowance paid.

Experience in the disability support sector or NDIS-related roles is highly desirable. Relevant qualifications in community services, disability support, or a related field are preferred but not mandatory.
