BE THE DRIVER OF TRUE CLIENT FOCUSED SERVICE AND CARE.
At Assist Right Supports, we are passionate about providing high-quality, person-centred support to individuals living with disability. We are currently seeking a NDIS Client Lead who thrives on connection, collaboration, and delivering meaningful outcomes for our clients, families, and our staff.
This role is uniquely positioned – part of the leadership team based in our office, and part of the on-the-ground support team, working alongside our support workers and clients in their homes and communities, driving true client centred care and taking charge of the outcomes that follow.
About the Role:
As a NDIS Client Lead, you will oversee the implementation and continuous improvement of tailored client care plans aligned with NDIS goals, future planning for our clients, and ensuring that Assist Right Supports clients are getting the most person-centred care they deserve, that is values driven.
This hands-on leadership role includes:
Collaborating with Clients, House Leads and Support Workers to co-develop weekly routines, menu plans, and budgeting strategies.
Attending client allied health appointments and ensuring allied health recommendations are actioned and embedded into daily supports and care plans that reflect the people we are supporting.
Ensuring accurate and timely reporting, including shift notes, incident reviews, progress updates, and NDIS reporting requirements, with support of our directors.
Supporting a culture of accountability, empathy, and excellence, where our clients’ voices are at the centre of everything we do.
Facilitating team meetings with our Team Leaders, and monthly client and family meetings and progress reporting.
Acting as a key liaison between the Directors, support teams, and external stakeholders.
What We’re Looking For:
We want someone who is just as comfortable in a staff meeting as they are in a client’s kitchen. You’re someone who:
Has experience in disability services, mental health, allied health, or support coordination.
Brings energy, initiative, and organisation to day-to-day operations.
Understands NDIS processes, documentation, and the importance of individualised service delivery.
Is a confident communicator who can mentor, advocate, and problem-solve.
Leads by example and sees leadership as being in service to others.
Are not afraid to get onto shift and support our clients. Assist Right Supports are true believers in “ground up leadership” we do what our team do.
Can manage priorities across multiple clients, ensuring nothing falls through the cracks.
