About the Role:
We are seeking a passionate and dedicated Service Delivery Officer to join our team and complete a traineeship in Certificate 3 Community Services. In this role, you will work directly with NDIS participants, providing professional, effective, and timely support. You will play a crucial role in ensuring participants receive the services they need to live independently and participate fully in their communities.
Provide high-quality customer service, ensuring Participants are well-informed and supported throughout their time with Community Access Care
Collaborate with various stakeholders, including healthcare providers, support workers, and family members, to coordinate services to provide holistic person-centered care
Maintain accurate and up-to-date records of participant interactions and service agreements, including support with intake of new Participants
Provide effective and efficient support to the Service Delivery Team
Assist with new Support Team onboarding with inductions and buddy shifts
Contribute to monthly reporting, auditing, and data collection for Service Delivery
Assisting to ensure the consistent implementation of best practice strategies in the provision of support services to Participants and their families within the community
Requirements of the role:
Have true empathy & a positive attitude
Eligible to undertake a Certificate III Community Services through an Australian Traineeship
Previous experience in Community Services (preferred)
Strong written and oral communication skills including Microsoft Office product suite
Adhere to confidentiality, privacy and maintain professional boundaries at all times.

NDIS Worker Screening Check - No Card/No Start
Valid and current First Aid and CPR Certification
Must have a Divers Licence & Comprehensive Car Insurance
Must have completed the NDIS Worker Orientation Module & Supporting Safe and Enjoyable Meals Module
