Ultimate Health Aged Care is a well-established provider of aged care and support services, dedicated to enhancing the lives of elderly Australians through compassionate, client-centred care. As part of our continued growth, we are seeking an experienced Client Coordinator to join our team at our Greystanes facility.

Position Overview
We are looking for a skilled and detail-oriented Client Coordinator to review and manage the activities, services and care for clients individual needs. This role plays a crucial part in ensuring compliance, quality, and the smooth execution of service and government agreements across the organisation.

Key Responsibilities

Review, negotiate, and prepare the My Aged Care plans between our clients and the Australian government on behalf of the organisation.

Manage and maintain plan documentation and associated records.

Liaise with internal stakeholders and external contractors to ensure client’s plans are being executed accordingly.

Work with our domestic support, healthcare and community support stakeholders to ensure plan objectives are being met and adhered to.

Advise senior coordinator and management on plan and stakeholder compliance and feedback.

Monitor the performance of all stakeholders and contractors, including aged care workers and report on variations and compliance.

Prepare reports, submissions, and documentation for management and regulatory purposes to the Australian government.

Collect and analyse data for plan management and variations.

Requirements

Relevant qualifications in business, contract management, law, or a related field.

Minimum of 1 year experience in a similar role.

Strong knowledge of My Aged Care systems and procurement procedures.

Excellent written and verbal communication skills.

Strong attention to detail and problem-solving skills.

Ability to manage multiple tasks and meet deadlines.

Desirable

Previous experience in aged care, healthcare, or community services settings.

Familiarity with Australian industry regulations and compliance standards.
