About the Role
We’re looking for a compassionate and driven Care Manager to join our team in Birkdale. In this role, you’ll be the key point of contact for new and existing clients, managing their care journey from first enquiry through to ongoing support.
You'll play a hands-on role in care planning, coordination, and quality assurance, working closely with our office staff, caregivers, and external providers to deliver exceptional, tailored care. Key Responsibilities
Develop and manage personalized care plans aligned with client goals
Maintain open, respectful communication with clients, families, and team members
Formalize care agreements and ensure clear understanding of services and costs
Accurately document all client interactions and share relevant updates with the team
Conduct in-person reviews with clients and families to assess care needs and adjust plans as required
About You
A Cert III in Aged Care (or similar) is preferred but not essential
Experience working with seniors, ideally in aged care or home care
Familiarity with Home Care Packages is highly desirable
Proficiency with Microsoft Office—especially Excel, Word, and Outlook
Strong admin and time management skills
Excellent verbal and written communication
A flexible, organized multitasker who can adapt to shifting priorities
Must have a valid Australian driver’s license and the ability to travel across Brisbane South, Redlands, and Logan regions
Must have the legal right to work in Australia
