We help our clients (we call them Members) live vibrantly in their own homes by wrapping a care team around them and their families. About the Role
You'll be working as a Care Planner in our Welcome Team. This position is full-time, however we are open to the candidate being Brisbane based or interstate. This role plays an incredibly important role in ensuring no stone is left unturned during the onboarding process with Five Good Friends, and to set up our new members and their families for success.
The core responsibilities of the role include:
Maintain a highly developed working knowledge of legislation governing home and community care
Care Planning for new members of Five Good Friends
Liaise with allied health professionals and external service providers in the health care industry to build excellent working relationships in the interests of facilitating Member assistance
Liaise with other FGF teams to ensure a smooth transition into our service for our members - including workforce, finance, care management and third party ordering
Managing any questions, concerns or complaints raised throughout the process by stakeholders in a professional and understanding manner
Reviewing and updating initial care plans created through sign up process - ensuring quality care planning
Managing budgets - planning members care in alignment with funding
Applying for supplements if applicable to enrich members care
Arranging allied health services for members to ensure their safety in the home
Ensuring a smooth and happy journey into long term care / support management with Five Good Friends
Do you have what it takes?
Tertiary qualifications in the health field highly desirable (ie. Bachelor of Nursing/Diploma of Nursing)
Allied Health Professional (ie Social Worker, Mental Health Professional)
Previous case management / care planning experience
My Aged Care / HCP / CHSP / DVA / NDIS experience highly desirable
Operating with a working knowledge of the NDIS Practice Standards and Aged Care Quality Standards
Understanding of Home Care / NDIS funding
Ability to work independently and manage multiple tasks simultaneously.
Enjoys working within a team environment.
