Registered Nurse - Clinical Coordinator (Disability- Aged Care).

As a Registered Nurse-Clinical Coordinator, you'll play a crucial role in supporting our Disability Services Manager to provide outstanding care and clinical services to our mostly disability and select aged care clients. Your collaboration with the Services and Complex Care team will be key in fostering effective coordination and ensuring we deliver on a day-to-day basis, safe, high-quality, and exceptional customer service at the frontline.
In this role, your amazing people and communication skills will not only assist with quality assurance, team training, team collaboration and clinical tasks but also serve as a positive and professional ambassador for our organisation. Building strong relationships with both clients and team members will be central to your success.
Your primary focus will be to deliver exceptional frontline customer service to our existing clients, you'll also confidently work collaboratively with our new specialised ELITE SUPPORT TEAM, (SW and EN's), liaise with hospital transition teams, nurse navigators, allied health professionals, and will also assist with onboarding new clients across SEQ, as needed.

ILS Services will be asking you to do the following:
Support the Services Manager in their tasks
Provide frontline clinical safeguarding and expertise to ensure high-quality community nursing services, as required.
Collaborate with the Services, Complex Care and Clinical Teams
Conduct comprehensive health assessments to aid in the development and review of client care plans.
Coordinate with medical and allied health teams.
Perform various clinical tasks in the community as required
Monitor, observe, and maintain accurate records and reports.
Oversee client medication management.
Approach the role with a positive, friendly, ethical, and professional demeanour.
Adhere to stringent industry standards, regulations and codes of conduct with strong attention to detail.
Undertake frontline support shifts and care in emergencies
Be flexible and adaptable to the demands of clients and the team
Engage in training, upskilling, and professional development, to maintain registration
Participate in On Call (clinical only), as needed.
Provide training and coaching to Support Workers and EN team members as required.
Desired Skills & Experience
Exceptional communication written and oral skills
Previous community nursing, or disability rehabilitation nursing is highly regarded
Experience working with people with various disabilities, including TBI, SCI, ageing clients and mental health clients is highly regarded
Experience with using manual handling equipment is highly regarded
Ability to work autonomously and use your initiative and problems solving at the frontline
Ability to identify and report client care and clinical concerns to the Services and Clinical Manager
Strong relationship building and collaboration skills
Catheter management, Wound management, Medication management, Diabetes management, Bowel Therapy.
Understanding or training in trauma-informed care principles.
Experience working with diverse cultural groups.

Registration with AHPRA
Valid First Aid & CPR certification
Experience working with participants with complex clinical needs
Ability to work calmly under pressure.
Current valid Driver's License
Completion of NDIS Workers Screening Clearance and Orientation Module - or willing to complete
Ability to participate in out-of-hours activities as required to undertake the role, or support business activities
