Nurse Next Door is a certified Great Place to Work and is on the lookout for an experienced Community Care Coordinator for aged care and disability clients in their homes.
This role is located in our Hawthorn office, our Headquarters for Nurse Next Door Australia. The role requires daily travel to clients homes and includes the use of our iconic Pink Car!
Your Next Adventure
Reporting to our Clinical Operations Manager, your role will be managing your own client portfolio of both aged and disability clients. This includes but will not be limited to the following:
Working one on one with clients and their loved ones, focusing on their individual nursing needs and happiness
Leading a team or nurses and caregivers assigned to your clients
Competent in basic & complex would management
Competent in catheter insertion -SPC and IDC (both female & male)
Maintain meticulous care documentation records
Continuous quality improvement

Mandatory Requirements
Registered Nurse (AHPRA Registration required) with no conditions or limitations imposed on practice - minimum 5 years experience
Experience in either a Home Care setting or Aged Care facility is highly regarded
Current Victorian Drivers License
NDIS Workers Screening Check, or willing to obtain clearance
Working With Children Check - employee status , or willing to obtain
