Position Summary

St Catherine’s is seeking passionate and experienced Clinical Care Coordinators to oversee the day-to-day clinical operations of our aged care facility. This role provides leadership to care staff, ensures excellence in clinical care, manages compliance with aged care standards, and contributes to organisational funding through the AN-ACC model.

Key Responsibilities

Coordinate clinical care and oversee daily operations within the unit.

Lead, support and manage nursing and care teams.

Ensure care practices align with Aged Care Quality Standards and clinical best practice.

Manage AN-ACC assessments, classifications and documentation to optimise funding.

Maintain accurate and timely clinical records and ensure compliance with accreditation requirements.

Support staff education and promote a culture of continuous improvement.

Collaborate with the executive team on risk, incident, and quality management systems.

Key Selection Criteria

The successful applicant will demonstrate:

Previous experience in a clinical leadership or coordination role in residential aged care.

Strong leadership skills and the ability to manage staff through change with professionalism and empathy.

Advanced clinical assessment and decision-making skills.

Experience in dementia care and managing complex behaviours.

Proven ability to manage and maintain compliance with the Aged Care Quality and Safety Commission Standards.

Sound understanding and application of the AN-ACC funding model, including documentation requirements.

Excellent interpersonal, written and verbal communication skills.

Proficiency in problem-solving and a proactive, solution-oriented mindset.

Commitment to ongoing professional development and evidence-based practice.

Qualifications and Requirements

Essential:

Bachelor of Nursing with current registration through AHPRA.

Current National Police Check.

Fully vaccinated against COVID-19 in accordance with aged care mandates.

Demonstrated commitment to resident-centred care.

Sound working knowledge of AN-ACC.

High-level leadership and team management capabilities.

Desirable:

Membership or active participation in a relevant professional association (e.g., Royal College of Nursing Australia).
