This 12-month contract role offers the possibility of extension and provides the opportunity to support clients across aged care and disability services.

About the Role

As a Care Coordinator, you’ll be the vital link between our clients, their families, and our team of support workers. Your role will focus on delivering personalised, high-quality care that aligns with each client’s unique needs and preferences.

Key Responsibilities

Coordinate and schedule in-home care services

Match clients with support workers based on skills, availability, and compatibility

Maintain ongoing communication with clients, families, and staff

Monitor and update care plans to reflect clients’ evolving needs

Ensure compliance with NDIS and Aged Care Quality Standards

Provide exceptional customer service and maintain accurate records

What We’re Looking For

Minimum 1 year experience in a Care Coordinator role

Solid understanding of NDIS and Aged Care Quality Standards (preferred)

Excellent time management, organisation, and communication skills

A client-focused and empathetic approach

Experience with Visual Care Software (preferred but not required)

Relevant qualifications such as:

Certificate III or IV in Individual Support (Ageing or Disability), or

Diploma in Community Services

Nursing Diploma (Enrolled Nurse)
