About the Role
As a Clinical Care Partner, you’ll take on a dynamic dual role—providing expert nursing and clinical oversight while coordinating tailored Home Care Packages for a dedicated caseload. You’ll work closely with consumers, support staff, and external providers to ensure seamless service delivery and outstanding care outcomes.
Key Responsibilities
- Conduct nursing assessments and clinical reviews
- Oversee clinical service delivery and support team members
- Coordinate direct supports and brokered services
- Manage individual budgets and monitor expenditures
- Ensure compliance with Home Care Service Agreements
- Communicate effectively with consumers and stakeholders
- Maintain accurate records and report on program outcomes
You’ll be supported by a dedicated team and work closely with our Care Manager. A fully maintained, home-garaged RLOA vehicle (with fuel card) is provided for appointments and events.
About You
You’re a passionate and experienced Registered Nurse who thrives in a community care setting. You bring strong clinical skills, a consumer-focused approach, and a commitment to improving lives.
Essential Criteria:
- Bachelor of Nursing with current NMBA & AHPRA registration
- Experience in Community Aged Care/Home Care Package programs
- Solid understanding of aged care standards and reforms
- Strong assessment, problem-solving, and time management skills
- Ability to work independently and collaboratively
- Excellent communication and administrative skills
- Proficiency in electronic consumer management systems
- Current Driver’s License and ability to pass probity checks
