About the position

An opportunity exists within our team on a Permanent, Full Time basis for a Care Partner. Care Partners provide varying levels of care management and support for participants in receipt of Home Care Package funding to remain living independently in their own home. Care Managers work collaboratively with participants and stakeholders to facilitate effective participant focused care, supporting participants to optimise their quality of life and participation in the community. Care Partner's case management framework is centered around wellness and reablement and includes processes of engagement, assessment, budget management, planning, service delivery, monitoring, review, evaluation and referral.

Considering an individual’s circumstance and level of complexity, the role actively supports participants to -

develop, implement and manage a plan of supports to suit their individual needs

coordinate and monitor supports that enable greater independence across all domains of their life

connect with and participate in their communities

meet their goals and exercise choice that supports them in pursuing their own lifestyles.

The successful applicant must have experience exceptional communication skills, excellent organisational skills, the ability to problem solve, a high level of professionalism and demonstrate a high degree of participant focus.

Previous experience in a similar role is highly desired.

Current Driver’s License is essential. Employment is subject to a satisfactory Federal Police and NDIS worker screening check.
