This role demands strong, proven experience within the aged care sector. A deep understanding of home care implementation, case management, and the intricacies of residential aged care is not just beneficial—it’s essential. As a Care & Housing Coordinator, you will be instrumental in guiding older Australians—particularly those who may lack family support, face language barriers, live with cognitive decline, or are hesitant to seek help—through the process of securing appropriate care and housing. Your role will include interpreting the Aged Care Act, developing tailored care plans, and coordinating access to vital supports, including housing, health services, home care packages, and community programs.

Your work will not only promote stability, safety, and wellbeing but will also protect the dignity and independence of our clients. By reducing social isolation and advocating for individual needs, you will help older Australians live fuller, safer lives.
