Your new role:

The Care Partner will support Lumia Care Team with its growth and services while providing excellent customer service for Consumers and adhering to the Aged Care Standards for Consumers with Commonwealth Home Care Packages (HCP).

You will onboard and provide oversight of a caseload of Lumia Care consumers through your comprehensive assessment skills. Furthermore, you will utilise your extensive knowledge (training provided) of Commonwealth Home Care Packages to optimise a consumer's home care journey.

You will be a confident, friendly, approachable, and professional individual who will maintain the relationship and communication with the Clients and their families.

Experience/Skills Required:

Aged care/Home Care Package experience desirable but not essential. Full training to be provided.

You will be highly organised, motivated and have a passion for keeping our elders at home safely.

Well-developed communication and decision-making skills.

Your own vehicle and Drivers Licence.

Must be a Registered/Enrolled Nurse with minimum 2 years’ experience.
