Clinical Care Manager
Seeking a Care Manager to oversee all clinical operations at a Residential Aged Care Facility. You will directly report to the General Manager who requires assistance with all clinical matters. The position is incredibly busy and requires a person who has initiative and is committed to quality clinical care. There will also be another Care Manager to support you within this large facility.

Duties:

• Oversee the daily clinical operations of the care home
• Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
• Review clinical incidents, complaints and changes
• Lead compliance
• Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers
• Prepare and conduct clinical governance audits
• Oversee medication management in accordance with resident needs and legislative requirements
• Liaise with allied health, medical practitioners and other services to provide optimal resident care

Requirements:

• Current AHPRA as a Registered Nurse
• Post grad qualification in aged care highly regarded but not essential
• Experience managing a minimum of 30 beds
• Proven track record in delivering quality clinical care within an aged care or acute setting
• Excellent clinical assessment skills and care planning capabilities
• Highly effective communication, leadership and relationship management skills
• An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
