The Role

As an AOD Therapeutic Case Manager, you will champion a client-centred approach to recovery by crafting and delivering tailored case management strategies. With deep clinical insight and therapeutic expertise, you will empower individuals to actively engage in their journey toward wellbeing.

Your key responsibilities:

  • Lead and facilitate group module sessions for clients, fostering a supportive and engaging environment.
  • Conduct regular reviews of case management plans and participate in case coordination meetings with Client Outcome Facilitators to ensure the client's needs and objectives are accurately identified and addressed.
  • Prepare comprehensive case documentation, including notes, referrals, reviews, and case closure summaries.
  • Build and maintain professional relationships with both internal and external stakeholders, actively seeking opportunities to enhance and promote service delivery options.
  • Collaborate with the Program Manager to research, evaluate and update the weekly program to align with client needs and preferences.

This is a Permanent Full-time (38 hours per week) role based in Brisbane.

To be successful in the role, you will meet the following criteria:

  • Bachelor level in Community Mental Health, Alcohol and Other Drugs or related discipline such as Social Work, Psychology or Social Science or equivalent (required).
  • Demonstrated understanding of AOD and dual diagnosis treatment principles. Skills and experience in a range of AOD interventions relating to substance use (including assessment, brief intervention, risk assessment and management, family support, and case management/care coordination).
  • Demonstrated a high level of knowledge and understanding of the AOD treatment sector, the mental health sector and community support services.
  • Demonstrated service coordination practices with internal and external service providers (including primary health/medical sector) and the ability to provide holistic care for clients presenting with a broad range of substance use and psychosocial needs.
  • Understanding of harm minimisation and reduction principles and ability to work with a recovery-oriented model.
  • Understanding of and commitment to the philosophy of the St Vincent de Paul Society Queensland and an ability to work in accordance with policies and procedures to promote the values and mission of the Society.
