We’re currently seeking an Experienced SIL/Complex Case Manager to join our growing team. If you're passionate about making a tangible difference and have strong experience in coordinating high needs supports, we want to hear from you.
About the Role
As a SIL/Complex Case Manager, you will oversee and coordinate services for participants with complex needs, including psychosocial, behavioural, or medical complexities. This role involves collaboration with participants, families, support teams, and external stakeholders to ensure quality outcomes aligned with individual NDIS plans.
Key Responsibilities
Manage and coordinate SIL and complex support plans in line with NDIS funding and goals.
Conduct assessments, risk management, and develop tailored support plans.
Support frontline teams to ensure high-quality, person-centred service delivery.
Monitor participant progress and adapt plans as needs evolve.
Collaborate with multidisciplinary teams, healthcare providers, and external agencies.
Maintain accurate documentation and ensure compliance with NDIS Practice Standards.
About You
Minimum 2 years’ experience in SIL coordination or complex case management.
Strong understanding of the NDIS framework and supported accommodation models.
Experience working with individuals with complex support needs (e.g. behaviours of concern, mental health, forensic backgrounds).
Excellent communication, leadership, and stakeholder engagement skills.
Relevant qualification in Social Work, Disability, Community Services, or related field.
Current Driver’s Licence and NDIS Worker Screening Check (or willingness to obtain)
