ISCP Care Coordinator - ATSI Identified Position

IWC is an Aboriginal community-controlled and multi-accredited health and wellbeing organisation that delivers services without discrimination to Indigenous and non-Indigenous peoples in Bundaberg and the Wide Bay Burnett.

ABOUT THE POSITION:

The Indigenous Stepped Care Program (ISCP) Care Coordinator is responsible for supporting the Stepped Care Mental Health program within IWC. The ISCP will provide Aboriginal and Torres Strait Islanders with or at risk of Mental Health illness with supports tailored to their needs.

DUTIES:

Arranging services for clients in accordance with care plans and in consultation with Health professionals.

Preparing brokerage request forms and correspondence to key stakeholders, including the client and those involved in the client’s care.

Liaising with external stakeholders to promote referral pathways and to enhance services and supports for clients.

Planning and coordination of activities/work to achieve operational requirements.

SKILLS & EXPERIENCE

Attention to detail with a high level of accuracy.

Confident and professional phone manner.

High level of confidentiality.

Proficient in maintaining records and adapting to new systems.

KEY REQUIREMENTS:

Certificate III/IV in Primary Health Care or willing to obtain.

A current driver's licence is essential or ability to obtain.

A First Aid Certificate or ability to obtain.

A Blue Card (Working with Children Card QLD)* or ability to obtain.

A Federal Police Check or ability to obtain.

* No Card, No Start legislation means that the successful applicant will need to hold a valid blue card before they start work. IWC will cover the cost of this application for the successful candidate.

SALARY PACKAGE:

Your Salary Package is made up of the following: -

Salary Sacrifice Packaging valued up to $15,990 per annum – adding approximately $2,000 to your take-home pay (available after completion of probation).

Meals and Entertainment Package up to $2,600 per annum (available after completion of probation).

9-Day Fortnight (for full-time staff).

Superannuation valued at 12%.

Annual Leave Loading of 17.5%.
