About us
Hi there! We're Five Good Friends, and we're an approved aged care and registered NDIS service provider. We help our clients (we call them Members) live vibrantly in their own homes by wrapping a care team around them and their families. Our software coordinates in the background so our teams can do their best work. Our carers (we call the Helpers) play a key role in creating the magic for our members. Five Good Friends strives to form high-quality, long-lasting connections between Members and their Helpers.
About the Role
The purpose of this role, known internally as a Self-managed Care Navigator, is to ensure members and their Authorised Representatives are empowered and well-informed, enabling them to confidently navigate our Self-managed tools and make informed decisions about their services and purchases.
Support Members to self-manage their care by assessing needs, developing goal-focused Help Plans, and ensuring understanding of their funding and the Five Good Friends model.
Action Remote Care Monitoring alerts and observations, collaborating with Members, Helpers, and the Care Team to ensure timely responses and accurate documentation.
Maintain working knowledge of Home Care Package program requirements, Aged Care Quality Standards, and FGF policies and workflows.
Provide high-quality customer service, assisting with enquiries, onboarding, app use, and ongoing support for Members and their Authorised Representatives.
Match Members with suitable Helpers and support both parties to build strong, long-term relationships that deliver safe, personalised care.
Monitor Member wellbeing and risk, ensure effective care delivery, and liaise with health professionals and service providers as needed.
Do you have what it takes?
Qualifications in case management or disability, aged care, or the human services field.
Assessing, planning and coordinating care and support services for the aged or individuals with disability.
Using technology, e.g. computer based system solutions, app based programs, in the everyday work environment.
Experience working in multi-disciplinary teams.
Customer service.
Care coordination in a home care package program and/or NDIS funded support services - in the self managed environment would be highly regarded.
Operating with a working knowledge of the Aged Care Quality Standards.
Rostering experience would be a bonus.
