We're on the lookout for a remarkable individual brimming with enthusiasm and passion what they do. The role of a Client Care Manager is pivotal in ensuring the delivery of comprehensive, participant-centered care, managing care plans, and coordinating services within the community.

Key Responsibilities:

Care Coordination: Oversee the delivery of care services, ensuring participants receive timely and exceptional care that aligns with their care plan and goals.

Monitoring and Reporting: regularly monitoring participant’s health status and progress, providing clinical oversight, and liaising with health care providers and family members as needed.

Leadership and Supervision: Supervise and support a team of carers, ensuring they are delivering care to the highest standards, and providing guidance and training where necessary.

Compliance and Documentation: Ensure that all the care delivery complies with relevant regulations, standards, and best practices. Maintain accurate and up to date participant records and documentation.

Family Communication: Build and maintain strong relationships with clients’ families, providing them with regular updates on their loved one’s care and wellbeing.

Continual Improvement: Participate in ongoing professional development and contribute to the continual improvement of care practices within the company.

Mandatory Requirements:

5 years experience in case management within aged care services
