About the role

Angels in Aprons' is seeking a motivated and compassionate Care Coordinator to join our team on a contract casual basis. In this crucial role, you will be responsible for coordinating the delivery of high-quality care services to our elderly and disabled participants from our Head Office in Milton, Brisbane. With a focus on person-centred care, you will work closely with our participants, their families, and our Angels to support them to stay comfortably in the place they love most ... HOME!

What you'll be doing

Onboarding, and ongoing assessment of participant needs and developing individualised care plans to meet their unique requirements

Maintaining detailed records and ensuring compliance with all relevant regulations and policies

Work in a small, personable team reporting to the Care Team Leader

Work in the after-hours phone on a rotating roster (shared with 5 people) including evenings and weekends

What we're looking for

Someone who is available for an immediate start, and ready to hit the ground running!

Minimum 2 years' experience in a care coordination or case management role, preferably within the aged care or disability services sector

Relevant qualifications in a health or social services-related field, such as nursing (preferred), community services, or individual support

Strong interpersonal and communication skills, with the ability to build rapport with participants, and their families

Excellent organisational and time management skills, with the ability to prioritise tasks and work effectively under pressure, using client management software and other relevant technology
