WA Country Health Service (WACHS) are seeking suitably experienced Assessment Officers to join their Aged Care Assessment Team.

We are seeking to appoint 11.6FTE across all the regions, including the Central Office in Perth. The Assessment Officer is part of a multidisciplinary team within the Aged Care Assessment Program. The position conducts assessments in a client's home or via telephone using an assessment tool which allows you to develop support plans and recommend services for older people.

This is a Recruitment Pool for Fixed Term Full Time or Fixed Term Part Time appointment until 31/12/2025, with the possibility of extension(s) and/or permanency.
