About the Role:
As a Care Partner, you'll be at the heart of our mission, working closely with individuals receiving funded aged care services through Astley Care. You'll be responsible for care planning, service management, monitoring, review, evaluation, and providing support and education.
Key Responsibilities:
- Maintain an excellent knowledge of legislation, guidelines, and information relevant to service provision and funding.
- Identify, market to, and support individuals to consider aged care support at home services with Astley Care.
- Develop, monitor, and regularly review care and service plans that describe individuals' current needs, goals, and preferences.
- Develop and monitor participant services cost projections to ensure services and supports can be delivered within the available aged care funding.
- Identify when referral to other agencies/health professionals is appropriate and action within agreed timeframes.
- Monitor the suitability, services, and quality of third parties providing funded services to participants.
- Provide ongoing support and advice and act as the first point of contact to individuals, supporters of individuals (if any), and external providers.
- Advocate for vulnerable individuals to ensure their safety and well-being.
- Liaise with Astley Care staff delivering direct care services to identify any changes in the support needs of individuals.
- Actively participate in Multidisciplinary Team Reviews and implement agreed actions.
- Work collaboratively with the scheduling team to manage individuals' schedules.
- Ensure that all care management activities are recorded in the client management system.
- Manage and resolve client incidents, feedback, and hazards using the incident and feedback management system.
- Participate in out-of-hours on-call rotation and cover for other Care Partners as required.
Selection Criteria:
- A Certificate III or higher in Aged Care or a Diploma level or higher in Community Services or equivalent with a Certificate 4 in Aged Care or working towards one is preferred.
- Minimum 2 years' experience coordinating community or aged care services.
- Commitment to embrace Astley Care's vision to provide choice, dignity, and independence to seniors and carers in our community.
- Highly developed interpersonal and communication skills with the ability to engage effectively and compassionately with everyone.
- Solid time management, organisation, and prioritisation skills.
- Commitment to own training and professional development.
- Ability to problem-solve issues that may arise in a home care environment.
- Calm under pressure, thinks ahead, and develops contingencies.
- Strong levels of professionalism, initiative, and enthusiasm.
- Proficient at using IT systems, including MS Office, client management systems, and work-related apps.
- Commitment to the highest standards of client care with a demonstrated understanding of the Aged Care Quality Standards.
