Cherbourg Regional Aboriginal and Islander Community Controlled Health Service (CRAICCHS) is an Aboriginal and Torres Strait Islander Community Organisation. CRAICCHS aims to provide accessible, high-quality, community-controlled primary health care and social and emotional wellbeing services to the Aboriginal and Torres Strait Islander peoples of the South Burnett.
The Position:
The Integrated Health Facilitator leads a team of Allied Health Professionals, Clinicians and Support Officers. To ensure facilitation of care, aged care and chronic disease management programs are provided within a comprehensive range of allied health, social emotional and wellbeing, mental health, drug and alcohol services, aged care, NDIS, and health promotion to achieve better outcomes in early intervention, treatment and management in a Primary Health Care setting.
The Integrated Health Facilitator will provide coordination and appropriate support to ensure the team delivers a coordinated and multi-disciplinary approach, ensuring improved access to a joined up integrated approach working collaboratively and in partnership with the Primary Health Care Portfolio holders and Community Health Services Portfolio.
Key Duties:
Lead the Integrated Health Team in line with CRAICCHS policies and funding service agreements.
Manage and monitor program areas to ensure the achievement of KPIs, outputs, and outcomes as per funder guidelines.
Coordinate and develop the quality and quantity of service delivery to ensure best practice and evidence-based approaches.
Ensure that all referral portals are routinely monitored for incoming referrals, with referrals distributed to the appropriate area within CRAICCHS in a timely manner.
Enhance case management, including systems, processes and adherence to service agreements.
Plan, implement, monitor, and evaluate the team’s programs and activities.
Support community-developed strategies for health promotion, prevention, and early detection.
Support a collaborative CQI approach to service delivery within the Integrated Health Services team.
Ensure timely and accurate reporting to meet all program requirements.
Develop annual work plans and schedules with program goals, strategies, and targeted outcomes.
Facilitate staff development and professional education opportunities for team members.
Undertake other duties as directed by the Executive Manager of Primary Health Care Services.
Key Requirements:
Essential
Diploma or extensive experience in Primary Health Care or a related field.
Demonstrated knowledge of comprehensive Primary Health Care principles, KPI's and social determinants in a regional Aboriginal community setting.
Strong leadership skills with experience managing a multidisciplinary team and fostering continuous improvement.
Knowledge of systemic issues affecting Aboriginal and Torres Strait Islander people and commitment to culturally safe engagement.
Ability to provide clear visionary leadership, encourage team development, and maintain a professional and approachable manner.
Current First Aid and CPR training, or ability to obtain.
Desirable:
Competency in Microsoft Office, business technology, medical software (Best Practice), and client management systems.
Qualifications in community services, health, social science, project management, or related fields.
