Location: Cloverdale, Perth WA
This is a part-time position (22.8 hours), leading to a full-time role for the right candidate.
Package (Annual): $55,000 - $60,000 per annum (pro-rata basis), + 12% Super.

About the Role
As the Support Coordinator at The Star Care Group, you will serve as the primary liaison, delivering specified individual services in accordance with approved and funded NDIS plans. This role involves coordinating various supports to meet client needs, assisting individuals in accessing NDIS funding packages, and primarily providing psychosocial support.
Support coordinators are tasked with guiding individuals with disability through NDIS eligibility processes, facilitating the development of individualized funding packages, and crucially, delivering services and support outlined in these packages.
The successful candidate will:
Submit case notes, records, and client information promptly to facilitate responsive service management, including NDIS invoicing and reporting on Funding Packages.
Provide Support Coordination and other services to clients participating in The Star Care Group's psychosocial support programs.
Educate, inform, and assist clients in determining NDIS eligibility and developing individualized funding packages.
Foster meaningful relationships with clients, their families, caregivers, and other support networks.
regularly check with participants to make sure their supports are still meeting their needs
Plan reassessment and evaluation report should be completed before the participant’s next plan reassessment.
Develop, monitor, and review NDIS Individualized Funding Plans and psychosocial support action plans for clients, maintaining regular communication to ensure personalized care.
Requirements include:
Qualification: Bachelors or above in Social work, Community Services, mental health, psychology or other relevant qualification.
Current National Police Clearance.
NDIS Worker Screening Check.
Working with Children Check.
Australian Citizenship or full working rights to live and work in Australia.
Valid Driver’s License
Covid-19 Vaccination
