Community Living Options (CLO) is a leading NDIS registered disability service provider.
We are a values-focused organisation, who put the people we support first. Established in 1982 by a group of local parents who wanted respite and accommodation for their adult children, we have expanded exponentially because our participants love our services.
At Community Living Options we are dedicated to providing an inclusive environment where people are valued and supported. We welcome applicants with lived experience, all abilities, cultural background, gender, sexuality, age and spirituality.
About the Role
We are seeking a passionate and experienced Service Coordinator to provide day-to-day leadership and direction to teams of Person-Centred Active Support Workers within our Forensic and Specialist Transitions service. In this role, you will play a central part in driving meaningful impact by mentoring and inspiring frontline staff to deliver high-quality, responsive services.
You will support people with complex psychosocial and forensic disability needs by enabling opportunities, fostering independence, and encouraging meaningful community connections. Working under the guidance of the Operations Manager, and in collaboration with participants, families, guardians, and other key stakeholders, you will help co-design and implement therapeutic, person-centred supports. You will also contribute to service evaluation to ensure effectiveness and positive participant outcomes.
Based in Edwardstown, SA, this role requires regular home visits to support frontline teams, monitor service quality, check participant wellbeing and safety, and complete site audits.
Key Responsibilities
Coordinate responsive supports for NDIS participants with complex psychosocial and forensic disability needs, helping them work toward their individual goals.
Conduct regular and timely reviews of supports to maintain up-to-date knowledge of each participant’s changing needs and ensure continuity of care.
Act as the first point of contact for participants and staff, fostering strong, respectful relationships with participants, families, teams, and external stakeholders.
Carry out regular home visits to ensure participant goals and needs are being met by well-supported, capable staff teams.
Conduct regular service quality and safety audits to uphold standards.
Ensure participant records, files, and support plans are accurate, up-to-date, and maintained in a timely manner.
Champion CLO’s values, focusing on participant and staff engagement, health, safety, and wellbeing.
Address concerns and service issues promptly, escalating as needed to ensure satisfaction and appropriate responses.
Lead and coach person-centred active support teams through ongoing feedback, supervision, and performance management—including team meetings and annual reviews.
Develop and plan rosters, ensuring appropriate matching of support staff to participants based on skills, risk factors, and preferences.

Proven experience coordinating supports for individuals with psychosocial and/or forensic disability support needs.
Strong understanding of therapeutic service models, trauma-informed practices, and positive behaviour support.
Demonstrated commitment to person-centred, inclusive, and rights-based support approaches.
Excellent communication skills, along with strong administrative, multi-tasking, and problem-solving abilities.
Experience in independently managing site operations and service delivery, aligned with organisational policies and procedures.
Ability to lead by example, inspire trust and high performance within teams.
Proficiency in Microsoft Office and administrative systems that support effective service delivery.
