Unique Minds Consultancy are seeking a dynamic staff member who is passionate about making a difference in the lives of our clients who can provide high-quality, person-centered support coordination, specialist support coordination and administration support in line with NDIS frameworks. We are looking for someone who is experienced with all things NDIS, who can build and maintain strong relationships with participants, families and providers.

Requirements:
· A minimum of 3 years' experience in a Support Coordination or similar NDIS-facing role
· Extensive knowledge of the NDIS standards
· Person-centered practice
· High level interpersonal, written and verbal communication skills
· Excellent organisation and time management skills.
· Experience preparing NDIS documentation
· Prior experience using Flow Logic would be an advantage, but not essential.
· Proficient in Microsoft Office Suite
· Experience with NDIS reporting platforms (MyPlace Portal, PRODA)
· Must have full working rights in Australia
· Eligible to work with people with disability in Queensland
About the role:
This is a Permanent Part time Role with the possibility of increase to Full Time. Working Hours are Negotiable.

· Restrictive Practice and Incident Reporting
· Service Agreements
· Funding and Financial Tracking
· Progress Reports and Compliance Monitoring
· Audit Preparation and compliance
· Launch of UMC Support Coordination – promoting UMC support coordination and growing the client base
Core Responsibilities & Task List
1. Client Data Entry & Management
· Enter all NDIA clients into the PASE platform upon intake.
· Enter all PBS clients with interim plans into the Proda platform.
· Ensure Flowlogic contains complete and up-to-date plan funding details for each client.
· Monitor and reconcile data across platforms: Flowlogic, PASE, MyPlace, and the payroll system.
2. Restrictive Practice & Incident Reporting
· Identify PBS clients with restrictive practices.
· Submit required documentation to the NDIS Quality and Safeguards Commission.
· Include a Plan to Reduce Restrictive Practice for each instance.
· Monitor and report all incidents involving:
Ø Children with restrictive practices.
Ø Children under any statutory child protection orders.
Ø Log and report these via Proda as per commission requirements.
3. Service Agreements
· Draft, send, and manage Service Agreements for all NDIS participants.
· Monitor expiry dates of all agreements.
· Ensure timely renewal and compliance with funding periods.
· Liaise with coordinators and families for signing and return of agreements.
4. Funding & Financial Tracking
· Input and track individual participant NDIS plan funding in Flowlogic.
· Monitor usage and flag underspend or overspend risks.
· Assist finance with reconciliation tasks by integrating data from:
Ø Flowlogic
Ø PASE
Ø MyPlace portal
Ø Payroll systems
5. Management of NDIS Support Clients
· Maintain up-to-date rosters for each supported accommodation house and participant.
· Coordinate with team leaders to ensure rosters are:
Ø NDIS-compliant
Ø Aligned with individual plans and approved support hours
6. Progress Reports & Compliance Monitoring
· Track and record report due dates:
Ø Progress reports
Ø Review reports
Ø End-of-plan summaries
Ø Ensure timely submissions to meet audit compliance requirements.
Ø Maintain central audit folders and ensure all data is audit-ready at all times.
7. Audit Preparation & Compliance
· Maintain documentation required for NDIS mid-term and certification audits.
· Prepare reports, evidence folders, and ensure platform data is up to date.
· Coordinate with clinical and service teams to collect and file:
· Risk assessments
· Behavior Support Plans
· Training records
· Restrictive Practice reporting evidence
· Provide administrative support to practitioners (PBS, OT, Psych).
· Schedule plan review meetings and reminders for staff and clients.
· Support with onboarding documentation for new participants.
· Maintain confidentiality and privacy compliance with NDIS Practice Standards.
