Sub-contracting position - Billed hourly rate of $60 per hour, 30 hours per week

At YOD Reflections, we are a NDIS Registered company, committed to providing Dementia specific, quality support co-ordination to NDIS participants with Younger Onset Dementia.

We walk beside our participants with consistency, understanding and compassion and use our extensive dementia knowledge and connections to assist in connecting them with Dementia trained service providers and assist in developing and maintaining strong relationships that ensure the support they receive is appropriate to their personal needs.

We are seeking an individual with strong NDIS knowledge and dementia experience (preferred), who shares our values and holds a genuine desire to make a difference. We are looking for a motivated and professional person who is self-driven and can work autonomously from home to join our team.

Ongoing training is provided to the team. Monthly team meetings and individual supervision are included. Supporting people with YOD can be is very emotive and self-care is essential. Counselling experience is helpful as participants are experiencing grief and loss during their dementia journey.

This is a subcontracting role, and we offer an attractive remuneration structure including Superannuation, ongoing training, supervision, and support.

You will be provided with weekly on-the-job mentoring, monthly team and supervision meetings and access to online training that you can utilise to increase your expertise in this role

You will have a supportive and passionate team to work alongside who possess a range of qualifications and experience in the NDIS and YOD Dementia field to support you in your professional growth and development

Position Objectives and Responsibilities

Service Delivery

Build capacity in people living with YOD dementia and their families to understand the NDIS and implement supports and services within the budget of their NDIS plan.
Empower people living with YOD and their families to express the level of require support they need to achieve their goals.
Assist people to connect with and choose supports in their local community to reach their goals.
Work with NDIS and mainstream services to meet client goals as identified in their NDIS plan.
Ensure all CRM data entry is accurate, of a high standard and is completed as service is provided.
Complete Support Co-ordination activities as outlined in YOD Reflections Resources and Guidelines.
Educate and promote awareness of YOD client needs when working with service providers and health services.
Work in the spirit of collaboration and connect with local sectors to increase opportunities for YOD clients to engage in activities within their local community.

Teamwork and Communication

Collaborate with team members to ensure integrated and co-ordinated approaches to addressing the needs of people living with YOD.
Demonstrate a high standard of professional and personal behaviour consistent with company values.
Maintain positive team relationships and actively participate in staff meetings.
Establish and maintain collaborative and positive working relationships with external stakeholders.
Promote YOD Reflections a centre of excellence for support, education and resources for people living with YOD, their families and carers.

Administration and Documentation

Ensure timely and accurate documentation of client activity and other appropriate information in line with professional and legal standards using prescribed databases. Complete electronic data recording of client and service information to assist with NDIS planning, claiming, and reporting requirements.

Quality

Maintain confidentiality of information pertaining to clients, carers, staff and YOD Reflections. Participate in regular supervision, support sessions and relevant Professional Development. Deliver and coordinate services that comply with standards, regulatory requirements and policies, eg NDIS Quality and Safeguarding Framework, National Disability Standards etc. Ensure that each client is provided with the opportunity to evaluate the experience of the support they receive from YOD Reflections as outlined in the Monitoring and Evaluation Framework Policy. Develop a continuous improvement model where opportunities to improve service delivery are identified and discussed with relevant staff members. Participate in professional networks as appropriate to support professional development/peer support to maintain connections with other service providers.

Model the organisations values and play a role in raising the profile of these values and associated behaviours across the organisation. Demonstrate a strong commitment to a quality culture, implementing standards of excellence and a continuous improvement business focus. Support a strong safety culture by ensuring all work activities are performed in compliance with the organisations Work, Health and Safety Policy.

Qualifications

Relevant qualifications and/or experience in the delivery of a community, health or disability-based service. NDIS Support Coordination experience is essential.

Skills and knowledge

Demonstrated experience/understanding of the complexities experienced by people living with YOD, their carers and families.
Experience in the delivery of Co-ordination of supports and an understanding of NDIS processes and systems.
Client engagement skills and the ability to support clients to achieve positive change and outcomes.
Proven capacity to problem solve and negotiate with others, including a flexible and create approach to overcoming perceived challenges.
Demonstrated high level of verbal, written and interpersonal communication.
Demonstrated ability to network with, consult and respond to the needs of a diverse group of stakeholders.
Ability to deliver and co-ordinate a high quality of service that comply with standards, regulatory requirements and policies.
Ability to work autonomously and as part of a team, and within established frameworks and timelines.
Commitment to supporting the needs and rights of people from special needs groups, wg: ATSI and CALD .
Experience with Microsoft Office suite and the use of electronic databases. We use Careview as our Crm system.

Conditions of Employment:

Twelve-month contract, with an option to extend 1-2 years following

A three-month qualifying period applies to all new staff members

Additional Requirements

The (prospective) employee will be required to:
Maintain a current driver’s license in NSW.
Provide evidence of entitlement to work in Australia, the maintenance of such entitlement being critical to continuance in role.
Undertake a Police Check prior to being offered the position.
Be flexible in work hours at times to meet the reasonable demands of the position.
Have own secure laptop, mobile phone, ABN and insurance.
Be willing to undertake travel as may be required with the position.
NDIS workers screening check certificate.
