About the Role:
- Permanent full-time position working a 7-day rotating roster within the Emergency Department (ED) and Intensive Care Unit (ICU) team, based in the inpatient mental health service. Provide advanced social work interventions within an acute multidisciplinary team.
- Participate in the 24-hour on-call social work crisis service, delivering timely support and interventions outside standard hours.
- Conduct comprehensive psychosocial and risk assessments to inform person-centred care, recovery-oriented practice, and clinical decision-making across acute inpatient mental health and complex medical settings.
- Lead care coordination and discharge planning by collaborating with hospital teams, community services, and external agencies to ensure safe patient transitions, including arranging accommodation and ongoing support.
- Deliver therapeutic counselling, advocacy, and clinical interventions addressing psychosocial impacts of mental illness; manage complex applications to government and non-government bodies such as NCAT, NDIS, and Housing NSW.
- Provide clinical supervision, education, and consultation to staff and students, and contribute to quality improvement, research, and professional development aligned with St Vincent's Hospital's mission and policies.
About You:
- Qualified Social Worker with a degree in Social Work, eligible for membership with the Australian Association of Social Workers (AASW), with advanced clinical expertise in mental health, including crisis intervention, psychosocial assessment, short-term solution-focused and trauma-informed therapeutic interventions, case management, and discharge planning, all delivered within a person-centred and strengths-based framework.
- Demonstrated knowledge and experience working with complex issues such as drug and alcohol use, domestic violence, child protection, victims of crime, trauma, homelessness, sudden death, and coronial processes.
- Recognised clinical leader providing expert social work advice in multidisciplinary teams, skilled in managing high-demand critical care environments, promoting consumer participation in decision-making, and leading service improvements to enhance patient care and outcomes.
- Excellent interpersonal, communication, negotiation, and organisational skills, with a commitment to collaborative team practice, professional development, and mentoring junior staff and students through supervision and education.
- Willingness to participate in a seven-day rotating roster
- Ability to contribute to the 24-hour on-call social work crisis service (5pm-8am shifts), contributing to a comprehensive, responsive social work service.
