The Role
We are seeking a friendly and proactive Intake Officer to join our growing team. This role is crucial in welcoming new Participants into our services and ensuring a smooth, supportive onboarding process. You'll be the first point of contact for many families and individuals, playing a key role in setting the tone for their experience with HCCS.
Tasks & Responsibilities:
Manage incoming referrals and enquiries from Participants, families, support coordinators, and external stakeholders.
Coordinate the intake process from initial contact through to service commencement.
Conduct intake meetings to gather essential participant information and ensure eligibility for services.
Liaise with internal departments (e.g. rostering, service delivery, compliance) to facilitate smooth onboarding.
Maintain accurate records and documentation in line with NDIS and organisational requirements.
Communicate clearly and compassionately with Participants and their stakeholders and representatives, ensuring a high level of customer service.
Assist in preparing and finalising service agreements, ensuring participants are fully informed about their rights, responsibilities, and the scope of services provided
Work closely with support coordinators and external providers to ensure a smooth intake process and that all necessary supports are in place for the participant’s transition into services
Requirements:
Experience in the NDIS, disability services, or community services sector is essential.
Previous experience in Support Coordination is highly desirable.
Strong communication, interpersonal, and organisational skills.
Proficiency in Microsoft Office and client management systems (CRM, CMS or similar).
Current NDIS Worker Screening Check and Blue Card (or willingness to obtain).
Valid driver’s licence and access to a reliable vehicle.
