The opportunity
Vacenti is seeking a Full time Clinical Manager to join the clinical team at our Portofino facility, located at Hamilton, QLD. Reporting to, and working closely with the Facility Manager, the Clinical Manager will ensure that our residents receive the best possible care and services. The Clinical Manager also provides expert clinical guidance and effectively manages clinical and lifestyle systems and processes to promote resident’s safety, well-being, and quality of life.
Your main responsibilities will include:
Monitor and assess the overall health and well-being of elderly residents.
Conduct regular assessments of residents' health and care needs.
Use data to identify trends and areas for improvement.
Work closely with healthcare professionals to develop and implement care plans.
Ensure that care plans are tailored to individual resident needs.
Provide leadership and guidance to care clinical staff at our facilities.
Stay informed about the changes to regulations and standards relating to aged care and ensure that the facility complies with all regulatory requirements.
Conduct regular audits to assess and maintain compliance.
Lead initiatives to improve the quality of care provided to residents using best practices and evidence-based care approaches.
Communicate effectively to residents, families and stakeholders and address any concerns or feedback.
Monitor and ensure that all staff comply with medication administration processes, protocols, and policies.
Collaborate with other departments, such as allied health practitioners to enhance the overall well-being of residents.
To be successful in this position you will:
Be a confident and capable clinician with a passion for aged care.
Have highly developed case management and problem-solving skills are essential, along with your ability to communicate effectively with staff, residents, families, and other stakeholders.
Be professional, patient, flexible and willing to go the extra mile to provide the support needed by residents and their families.
Have relevant experience in a senior nursing position with a sound understanding of AN-ACC and the Aged Care Accreditation requirements.
Have current registration with AHPRA and a minimum of 5 years’ experience in Aged Care is preferred.
To be considered for this opportunity you must have the right to live and work in Australia. If you are successful, you will be required to obtain a national police clearance prior to commencement.
