We are looking for a Clinical Care Manager to join our team at IWA. The Clinical Care Manager provide clinical leadership of the clinical team to ensure optimal care and services for ages care and disability. The role ensures the continued success of home care programs and overall team coordination at IWA Australia. The clinical Care Manager will be reporting to Chief Operating Officer.

As a Clinical Care Manager at IWA Australia, your main tasks and responsibilities will be:

Clinical Care management

Ensure the provision of effective care that is based within accepted contemporary health care practices.

Effective leadership of the Clinical Care staff, ensuring competencies and performance standards are met and maintained at all times

Track and oversee the Clinical Staff KPI monitoring systems

Overview and management of the Salam Respite Centre

Ensure aged care case management documentation meets professional and regulatory requirements

Data Management and Review:

Effectively collate clinical statistics and data in order to analyse and identify clinical trends, causes and outcomes.

Conduct regular clinical meetings and provide updates to executive management

Staff recruitment and Management

In conjunction with the NBSM oversee the recruitment process for clinical care staff across branches and Salam Respite Centre (SRC) staff

Assess and review KPIs of clinical staff and Salam Respite Centre (SRC) Staff

Conduct performance appraisals of clinical care staff

Ensure sufficient and suitable Clinical care Staff to support consumers and SRC staff

Risk Management/Leave Management

Manage the process of risk managing roles when staff members are sick or on leave.

Identify suitable replacements to ensure continuity of service to clients.

Salam Respite Centre

To effectively oversee and monitor the SRC’s intake process, case management systems and maintenance of care documentation

Ensure incident reports are completed as required

Ensure implementation of policies and procedures including medication management

Ensure appropriate staffing of SRC

Compliance:

Ensure compliance with all relevant agreements, standards, policies and procedures

Manage key performance indicators around service performance and delivery to ensure quality outcomes for programs

Ensure accurate advice is provided to care partners regarding inclusions and exclusions

Quality Assurance and continuous improvement

Develop and implement a clinical governance framework

Monitor the quality of clinical services (including brokerage) to ensure continuous improvement

Record and follow up on clinical incidents / complaints as required.

Monitor and implement clinical incidents and trends and report to the board

Ensure all compliance standards are met

Undertake audits relevant to Clinical care and SRC and analyse the outcomes

Ensure compliance with relevant Accreditation standards as they apply within the area of responsibility.

Monitor SRC resident satisfaction with the provision of planned care through relevant surveys and processes.

Contribute to policy/procedure development and initiate reviews when appropriate

Reporting:

Attend and provide reports to the Quality Assurance Committee (QAC)

Provide regular updates to upper management of any operational issues or emerging trends that impact the operations or quality of IWA’s aged care and NDIS program
