About the role

Join the Open Sky Disability Services' team as a part-time NDIS Support Coordinator based in Maylands, WA. In this critical role, you will work closely with participants to coordinate and facilitate the delivery of their National Disability Insurance Scheme (NDIS) plans, ensuring they receive the best possible support and achieve their individual goals. The role would be a minimum of 30 hours per week, with the potential to increase hours depending on suitability and interest.

What you'll be doing

Assist participants to understand and implement their NDIS plans

Coordinate supports and services, including mainstream, informal, and community supports

Identify service gaps and work collaboratively to resolve issues

Build participants’ capacity to exercise choice and control

Facilitate referrals and assist in engaging appropriate providers

Monitor the effectiveness of current supports and report progress toward outcomes

Support participants in preparing for plan reviews, including gathering progress reports

Maintain accurate records, service notes, and documentation in line with NDIS requirements

Uphold NDIS Practice Standards and Open Sky’s policies and procedures

Respond to incidents and escalate risks or safeguarding issues appropriately

What we're looking for

Minimum of 1–2 years of experience in NDIS support coordination or similar role

Sound knowledge of the NDIS framework and Price Guide

Strong understanding of psychosocial disability, intellectual disability, and complex support needs

Excellent interpersonal and communication skills

High level of organisation, documentation, and time management

Ability to work independently and manage a caseload

Familiarity with CRMs and digital case notes

Current driver's license and access to a reliable vehicle
