About the role
As an NDIS Support Coordinator with Little Blue Wren Services', you will play a vital role in empowering individuals with disabilities to access the support and services they need to live independent, fulfilling lives. With our head office based in Tamworth, NSW, this full-time position involves coordinating and connecting participants with appropriate service providers, ensuring they receive the personalised care and assistance required to achieve their goals, however the role itself is open to local and non-local applicants.

What you'll be doing

Develop and implement tailored support plans for NDIS participants, identifying their unique needs and coordinating the delivery of appropriate services

Liaise with a diverse network of service providers, including allied health professionals, to ensure seamless service delivery for participants

Monitor and review participant progress, adjusting support plans as required to optimise outcomes

Advocate on behalf of participants, ensuring their rights are protected and their voices are heard

Maintain detailed records and documentation to comply with NDIS guidelines and reporting requirements

Collaborate with the broader team to continuously improve service delivery and participant experiences

What we're looking for

Relevant tertiary qualifications in a human services or allied health field, such as social work, occupational therapy or disability studies

Demonstrated experience in disability support coordination, case management or a similar role, with a deep understanding of the NDIS and its processes

Excellent communication and interpersonal skills, with the ability to effectively liaise with a diverse range of stakeholders

Strong problem-solving and decision-making abilities, with a focus on participant-centred, compassionate care

Commitment to continuous learning and professional development to stay up-to-date with industry best practices

Willingness to travel within the local area to meet with participants and service providers as required
