Position Overview:
We are currently seeking a detail-oriented, compassionate, and motivated Service Delivery Coordinator to join our growing team. The successful candidate will play a vital role in supporting disability services participants to implement their plans, build capacity, and navigate complex service systems. This is a highly rewarding role that requires both strong interpersonal and organisational skills.
Key Responsibilities:
* Promote Willowbridge Care disability services to the market. This may involve delivering presentations to various organizations such as Local Area Coordinators and support groups including participants and nominees.
* As a Service Delivery Coordinator for Willowbridge Care , you will be part of the management team assisting with managing staff, rostering and liaising with participants and stakeholders.
* Assist participants in understanding and activating their disability support plans.
* Coordinate access to funded, mainstream, and community supports.
* Facilitate service. agreements, manage bookings, and support budget allocations.
* Monitor the effectiveness of supports and regularly review participant progress.
* Prepare documentation and outcome reports for plan reviews.
* Identify risks and implement strategies to mitigate service gaps.
* Build and maintain positive working relationships with participants, families, providers, plan managers, and other stakeholders.
* Promote participant choice and control, advocating for their rights and needs.
* Maintain accurate, timely case notes and administrative records in line with compliance and confidentiality standards.
* Attend relevant training and meetings to ensure continuous professional development.
* Keep up to date with disability operational guidelines and relevant sector changes.
Essential Criteria:
* Minimum 2 years of experience in a similar role within the disability or community services sector.
* Demonstrated understanding of the disability services framework and support coordination functions.
* Strong knowledge of local community resources coordinators and disability support coordinators.
* Proven ability to work with people with disability, including those with psychosocial, intellectual, or complex support needs.
* High level of professionalism, discretion, and emotional intelligence.
* Excellent verbal and written communication skills, including report writing.
* Advanced organisational, time-management, and problem-solving skills.
* Ability to work both independently and collaboratively within a team.
* Qualifications in Social Work, Psychology, Allied Health, Community. Services, or a related field
* Current Australian Driver’s License and reliable vehicle.
* Current NDIS Worker Screening Check (or willingness to obtain).
* Proficient in the use of case management software and Microsoft Office suite.
* Current First Aid and CPR certification (or willingness to obtain).
* Working with Children Check (if applicable).
Desirable Criteria:
* Lived experience or additional language skills.
* Knowledge of trauma-informed practice.
* Experience in culturally safe practice with Aboriginal, Torres Strait Islander or CALD communities.
* Training in mental health or psychosocial disability support.
