The Service Coordinator/Team Leader position involves working as part of a team and is responsible for rostering and scheduling of shifts for NDIS participants and Support Workers. The position includes matching of Support Workers to participants based on the needs of the individual.
The Service Coordinator/Team Leader is also responsible for overseeing the work of the Support Workers to ensure that they are adhering to all relevant service practices including keeping notes up-to-date, incident reporting and maintaining availability in the Customer Record Management application.
Other duties for this role include timesheet approval and completing risk assessments and requests for services. The successful applicant will also be required to be available to be part of an on-call after hours roster.
ESSENTIAL CRITERIA
Minimum of two years experience in same or equivalent role.
Good understanding of risk assessment.
Effective written and verbal communication skills.
Ability to work effectively as part of a team.
Competent computer skills and experience in Microsoft Office and/or specialised software such as Brevity.
Values driven approach to employment.
Effective time management skills.
Excellent attention to detail.
DESIRABLE CRITERIA
Understanding and experience in the NDIS sector.
Experience in developing service delivery plans.
Relevant qualifications.
Experience in working with people with a disability.
Car and current driver’s licence.
