The Opportunity
Digital health initiatives play a vital role in improving access to health and aged-care services through a more connected system of care.
Joining Murray PHN’s Digital Health and Connected Care team you will use your exceptional stakeholder engagement skills to support the implementation and monitoring of Residential Aged Care Homes (RACH) and primary health care digital health initiatives.
This is a parental leave cover contract offering an opportunity to develop professionally among a collaborative team while accessing genuinely flexible work.
Key Accountabilities
- Deliver the activities outlined within the Aged Care Activity Workplan to improve the health outcomes of senior Australians living in aged care homes and in the community.
- Facilitate digital health focused quality improvement initiatives, programs or projects, including but not limited to My Health Record, telehealth and clinical deterioration response, across primary care and aged care services
- Implement solutions and embed digital health technologies using a range of strategies including change management, project management, training, and in-scope technical support.
- Consult and maintain strong relationships with RACHs and other supporting health care providers across the catchment in collaboration with Murray PHN teams
About you
This position will appeal to a person passionate about digital health as a tool for improving person-centred health care.
Your interest in this field with be complimented by your ability to build effective, supportive relationships and build capability among organisations with varying levels of digital health maturity.
Key Capabilities
- Relevant degree qualification in Health, Business, or a related field and/or approximately five years in a position with similar responsibilities and experience.
- Experience of implementing digital health care systems services along with an understanding of health in a community setting and the barriers to achieving good health outcomes.
- High-level interpersonal and communication skills, including digital literacy, an ability to lead people through change, navigate business applications, My Health Record and telemedicine applications.
- Demonstrated understanding of project management principles and methodologies.
- A connection with the Murray PHN catchment, our values and vision.

About Murray Primary Health Network
Murray PHN (Primary Health Network) commenced on 1 July 2015 as a part of the national network of 31 PHNs across Australia, which replaced the Medicare Local program.
Murray PHN works closely with the health system to identify areas to improve, either through the better coordination and support of health services or by commissioning new services to address needs.
The catchment of Murray PHN is significant in both size and diversity. Spanning northern Victoria, and including Albury, NSW, the Murray PHN covers an area of almost 100,000 square km and it will impact health services that are accessed by more than 563,000 people.
The Australian Government has identified national objectives and priority areas for PHNs. These frame the focus of Murray PHN and will be guided by both community and clinical input to improve health outcomes.
