Lighthouse Connect Services is an NDIS-registered provider, providing Support Coordination and Plan Management services to clients with a disability and/or complex mental health diagnosis across Australia.
We are seeking passionate & experienced NDIS Support Coordinators to join our vibrant, hardworking, friendly, and committed team. This is a Part-time role, minimum 3 days a week (Days are flexible), with possible extension.
As a Support Coordinator, you will work along with our clients and their families, carers, and/or nominees to assist them to understand, implement and coordinate their NDIS Plan through a high-quality, person-centred service which optimizes the client's NDIS plan and funding. You will work closely with individuals to identify their goals and support them to access the services they need; ensuring the client’s choice, control and independence is at the heart of what you do.
Joining the team at an exciting development stage, your ability to work outside the box and be involved in networking events, meetings, and 1:1 engagement will help grow a thriving service offering for people living with disabilities.
What we'll need you to do
In this role, you will:
Establish rapport and build strong working relationships with clients and support them to develop an understanding of their plan and plan options.
Assist clients and (where applicable) their families, carers, and nominees to understand their rights and build their confidence and abilities to make informed decisions.
Assess the funding and services which are available for the client.
Work with the client, and their support network, to obtain quotes and assessments to meet the elements in the plan and provide maximum value.
Connect clients to a range of quality service providers, community, mainstream, and other services that provides dignified access and support.
Accurate record-keeping for all client and stakeholder interactions in the client database, including tracking of billable time.
Collaborate with NDIS, mainstream, and existing or potential supports in family, carers, and nominees to provide clients with options for linkages, referrals and supports for clients.
Regularly review and refine plans with clients and report outcomes to the NDIA through progress reports within the specified time frames.
High-level confidentiality, integrity, and professionalism displayed at all times.
Accept personal responsibility for the accurate completion of work and seek assistance when required.
Demonstrate reliability, punctuality, and professionalism in all work-related tasks.
There are, however, a few boxes you'll need to tick
Understanding of the NDIS including an understanding of the rules for reasonable and necessary supports.
Demonstrated ability to work in a person-centred environment to build capacity, choice, and control.
Understanding of the National Disability Insurance Scheme Act 2013, the National Standards of Disability, and relevant state-based legislation.
Ability to build and maintain rapport with a diverse range of clientele and stakeholders, including people living with a disability, cultural diversity, and language other than English, and be able to adjust ways of working to support best-practice inclusion and accessibility. Also being highly organised with the ability to juggle multiple requests.
Be welcoming, inclusive, and respectful of people from diverse groups and backgrounds.
Experience in community networking and contributing to organisational development.
Qualifications in community services, disability, or other health-related disciplines are essential.
National Criminal History Record Check (Police Clearance), NDIS Worker Screening Check, and Working with Children Check.
Possess a current open C-Class driver's license valid in Queensland.
Possession of or ability to gain a First Aid and CPR certificate.
Desirable:
Experience with Psychosocial and other disabilities will be highly desirable.
Experience in support coordination, local area coordination, mental health, and community services.
NDIS Worker Induction certificate.
Remuneration:
It is based on the ‘Social & Community Services Industry’ - Social and community services employee pay rates Level 3.2. Information regarding the rates and entitlements can be found on the Fair Works website.
