Ndislink Group (Moreable) Services is NDIS-registered provider committed to delivering high-quality, person-centered support to individuals living with disabilities. We are seeking a highly experienced and professional NDIS Support Coordinator to join our team and play a pivotal role in empowering participants to achieve their goals.

Key Responsibilities:

Plan Implementation:

Support participants in understanding and implementing their NDIS plans.

Coordinate all services and supports, ensuring seamless delivery.

Reporting & Compliance:

Complete and submit regular reports to the NDIS and other relevant stakeholders.

Monitor participant progress and address any issues proactively.

Plan Reviews & Advocacy:

Apply for plan review requests when necessary (e.g., changes in circumstances).

Attend plan review meetings with the NDIS and advocate effectively on behalf of participants.

Participant Engagement:

Meet participants in person regularly to build strong, trusting relationships.

Work collaboratively with families, carers, and other service providers.

Stakeholder Collaboration:

Join meetings with allied health professionals, service providers, and other relevant parties to ensure comprehensive support planning.

Facilitate connections with community, mainstream, and funded supports.

Position Overview:

As a NDIS Support Coordinator, you will provide expert guidance and coordination to NDIS participants, ensuring seamless implementation of their NDIS plans. You will navigate complex support environments with confidence and competence, delivering tailored, holistic solutions that promote independence and wellbeing.

Key Responsibilities:

Deliver Level 2 Support Coordination and/or Specialist Support Coordination (Level 3), supporting participants with complex needs.

Assist participants to understand, activate, and optimise their NDIS plans in alignment with their goals.

Build participants’ capacity for self-direction and independence through strong support networks.

Coordinate a range of supports, including mainstream, community, informal, and funded services.

Proactively monitor and review participant progress, resolving any issues and updating service plans as required.

Engage with stakeholders—participants, families, service providers, allied health professionals, and government bodies—to ensure integrated, effective support delivery.

Maintain compliance with all NDIS Quality and Safeguards Commission requirements, ensuring documentation and reporting are of the highest standard.

Actively contribute to continuous improvement processes within the organisation.
