Extended Families Australia has been supporting children and young people with disabilities and their families since 1978. Our support coordination service is delivered by a team of experienced staff from a culturally and linguistically diverse background. We work with children and adults, and their families, and we have strong multi-cultural links. Whilst our primary cohort is people with intellectual, physical, sensory, neurological disability and Autism, we work with an increasing number of people with mental illness.
As the Support Coordinator, you will support people with a disability and their families to navigate the NDIS, assess support options, connect with services and empower them to make decisions and choices regarding implementation of supports within their NDIS plan.
Who
We are seeking an experienced Mandarin and Cantonese speaking worker to complement our culturally sensitive team. You will have:
Previous experience as an NDIS support coordinator role or similar
Tri–lingual Mandarin, Cantonese and English; with good spoken and written English skills
Excellent communication, organisation, flexibility and empathy and ability to engage with families
A holistic practice orientation and an understanding of the various issues that may impact on a family such as mental health, child protection, relationship and socio-economic issues
Strong administration and computer skills
Ability to support children and adults with a disability and their families
Be culturally sensitive
Skills and experience in mental health support (preferred)
We are looking for people with qualifications in Social Work or equivalent.
The successful applicant will have relevant experience in and understanding of Disability Services, a current Victoria Driver’s license and access to a car, and a current satisfactory NDIS worker screening check and Working with Children Check.
This is a part time position, working 3-4 days a week but we are also open to a full-time role.
WHAT
Support Coordinators work with families, carers and NDIS participants to provide independent support, coordinate all aspects of their NDIS plan and to form links with a range of community, mainstream and funded support services. They help participants and their families to implement their NDIS Plan to achieve goals.
This includes assisting people to:
Source and assess support options and connect with services
Coordinate services
Implement, monitor and manage their NDIS plan
Build their skills and confidence
Respond and deal with any crisis
Ensure goals are met, report outcomes to the NDIS and re-develop goals prior to plan review
Orientation, training and ongoing support provided.
Where
You will be based at the Extended Families office in Box Hill, supporting families across Melbourne, primarily in the Eastern Region.
When
Most work is in business hours, although may include occasional after-school or evening visits to connect with relevant family members.
