Specialist Support Solutions are seeking an enthusiastic, Experienced Level 2 & 3 Support Coordinator to assist clients navigate the complexities of NDIS, to ensure that their plans are utilized effectively. Desired Requirements:

Ø Minimum 2 years experience in Support Coordinator or Case manager role

Ø A strong drive to contribute to positively impact the lives of our clients, and always go the extra mile

Ø Excellent communication skills and interpersonal skills

Ø A track record of success reaching productivity and activity targets

Ø Strong problem solving and critical thinking skills to address clients needs

Ø Ability to complete plan budgets and breakdown of NDIS plan

Ø Understanding of the NDIS Code of Conduct

Ø Experience in liaising with Allied Health Professionals, Stakeholders, Mental Health, Medical Professionals, Public Guardian and QCAT

Ø Relevant diploma/Tertiary qualification in a human services or allied health discipline is advantageous.

Your key responsibilities are:

Ø Empower clients with a deeper understanding of their plans and how to implement them

Ø Maintaining detailed case notes and administrative records to support NDIS reporting requirements

Ø Providing pre and post plan recommendations to assist clients utilize their NDIS plan

Ø Coordinate and build strong relationships with internal and external stakeholders

Ø Successfully meeting productivity and company targets

Ø Implement and Monitor budgets and support to ensure that funding expenditure is on track to the desired goal

Ø Conduct regular progress reviews of client’s plan, ensuring timely and accurate reporting to the NDIS
